What we collect
When you book an appointment with us, we collect contact details (name, address, phone, email, date of birth), Medicare and health-fund details where relevant, GP and referrer details, and the clinical information needed to provide your care.
How we use your information
We use your information to provide your treatment, communicate with you about appointments, claim through your health fund (HICAPS, Medicare, DVA, Workcover or NDIS as applicable), and meet our professional and legal obligations. We do not sell your information.
Where it's stored
Your clinical records are stored in Cliniko, our practice management software, which is hosted in Australia. Access is limited to clinicians and reception staff who need it to support your care.
Sharing your information
With your consent, we may share information with your GP, specialist, or other allied health practitioners involved in your care. We do not share your clinical information with anyone else without your written consent, except where we are legally required to do so.
Accessing or correcting your records
You have the right to access your clinical records and request corrections at any time. To do so, contact us by email at hello@fwdhealth.com.au. We may need to verify your identity before releasing records.
Concerns or complaints
If you have a concern about how we've handled your information, please raise it with us first by emailing the address above. If we can't resolve it together, you may contact the Office of the Australian Information Commissioner (OAIC).
